Many patients hear that one option is “natural” and the other is unsafe. What are synthetic hormones?

Bioidentical versus synthetic hormones: the differences that matter

FDA-approved medications offer standardized dosing
FDA-approved hormone medications undergo review for quality, safety, and effectiveness for their intended uses. Their dose and formulation are standardized, which gives physicians a more predictable starting point when prescribing and adjusting treatment.
These options include several forms of estradiol, progesterone, and testosterone therapy. Depending on the hormone and your needs, treatment may be delivered through creams, gels, patches, injections, tablets, or other approved routes. The best route is personal. A patient with skin sensitivity, digestive concerns, fluctuating symptoms, or specific cardiovascular risk factors may benefit from a different approach than someone else.
Compounded hormones can address specific needs, but require care
Compounded hormone therapy is prepared by a specialized pharmacy based on an individual prescription. It can be helpful when a patient needs a dosage or delivery method that is not commercially available, or when they cannot tolerate an ingredient in a standard product.
However, compounded preparations are not FDA-approved as individual products. Their potency and consistency may vary more than standardized medications, and they do not have the same level of large-scale clinical research behind each customized formula. That does not make them inherently inappropriate. It means they should be prescribed thoughtfully, obtained from a reputable compounding pharmacy, and monitored by an experienced physician.
Customized care is valuable when it solves a genuine clinical problem. Customization without a clear reason is not automatically more precise.
The hormone itself and the delivery method both influence risk
It is tempting to assume that bioidentical hormones carry no risk or that synthetic hormones are universally dangerous. Neither is accurate. Hormone therapy can affect clotting risk, breast and uterine tissue, mood, bleeding patterns, cholesterol, fluid retention, prostate health, fertility, and other areas depending on the medication used and the patient receiving it.
For women, estrogen treatment is considered differently depending on whether the uterus is present. In many cases, systemic estrogen requires adequate progestogen protection to reduce the risk of endometrial overgrowth. For men receiving testosterone replacement therapy, clinicians should consider symptoms, confirmed laboratory findings, prostate history, blood counts, cardiovascular health, sleep apnea, and future fertility goals.
Route matters, too. Transdermal estrogen, oral medication, injections, creams, pellets, and other forms do not behave identically in the body. A physician-guided plan considers more than a single lab result or a preferred product format.
Why symptoms should lead the conversation
Hormone levels change with age, stress, sleep, weight changes, illness, medications, and life stage. A number on a lab report can provide valuable information, but it should not be treated in isolation.
A woman in perimenopause may experience night sweats, disrupted sleep, brain fog, anxiety, vaginal dryness, and stubborn body-composition changes even when a single hormone test does not tell the full story. Testing may be recommended when clinically appropriate, with follow-up testing based on the treatment selected.

That may mean sleeping through the night more often, feeling more motivated to exercise, improving sexual wellness, reducing disruptive menopause symptoms, or supporting healthy body composition alongside nutrition and strength training.
Monitoring is not an afterthought. It is how treatment stays safe and effective over time. Depending on the therapy, follow-up may involve symptom reviews, blood pressure checks, laboratory testing, dose adjustments, and conversations about side effects. Patients should report unexpected bleeding, chest pain, shortness of breath, severe headaches, leg swelling, significant mood changes, or other concerning symptoms promptly.
